How to file a complaint with the Department of Health (DOH)
We encourage patients harmed by Medical Malpractice and/or those who have complaints and concerns regarding medical treatment received contact the Department of Health Medical Quality Assurance Division and submit a complaint explaining the circumstances. Complaints may be filed with the Department of Health Medical Quality Assurance by clicking the Florida Health Care Complaint Portal or you can download a complaint form. Once the complaint form is completed, it needs to be either emailed, faxed (850-488-0796) or mailed to:

How to Investigate Prior Medical Board Problems. You can start your investigation with the Florida Department of Health (DOH) by verifying the license of the care provider: Public Records License Investigation
Malpractice cases may or may not also involve a “Public Complaint” with the Florida Department of Health – Division of Medical Quality Assurance (“MQA”) and/or “Discipline on File” by the Medical Board.
